Alzheimer's Adult Day Services Program

The Alzheimer's Adult Day Services Program partially subsidizes the purchase of adult day care services for persons with Alzheimer's disease or a related dementia. The Department currently has letters of agreement with 45 adult day care centers in 19 counties. To be eligible for the program, an older adult must meet the following criteria:

  1.  Have a diagnosis, documented by a licensed physician, of Alzheimer's disease or a related dementia.
  2. Be routinely supervised by an informal caregiver.
  3. Reside in the community.
  4. Have documented evidence of eligibility according to income guidelines.
  5. Be a resident of the state of New Jersey.
